Can you make an example of the form without any validation needed to be done please?

Try this, though it may not be the issue - that is a hell of a form and a huge page, it is difficult to wade through it all. Rename the folder you have called "Forms and Confirmations" so that is has no gaps in the name. Call it something like forms_and_confirmations. Probably a good idea to leave out the caps also. Also if any of the JavaScript validation stuff is non-FP remove that for the moment also. The yellow fields are because you have the Google toolbar installed. It highlights the fields it can autofill in yellow.

Hi Ziggyzaz, Whew, that is a healthy form and Katherine is right about it being a bit much to wade through. I think however, before putting much more work into that form, it might be a good idea to publish a one or two field form to your site (no validation), setting the email destination only with the results set to Formatted text. See if this one will work before spending much more time with the other one. This way you can determine if the extensions and email transport are working properly.

You could also break that form up into smaller chunks across multiple pages.

A number of ways. Always attempt to break a big job down into smaller parts. Advice from others said that what you had was too much, that you should remove some of that, test it first to make sure it works, then put the remaining back in. I'm going along the same lines. This makes troubleshooting much easier, code easier to read, locations easier to find in your HTML. Today -industry standard when filling out forms is to do something like: Name and Address on page 1. Passwords, etc on page 2. Credit Card info on page 3. Etc....... Something like that. But that's only advice. You should do what you feel comfortable with. I was thinking simplistically.
