Being an .asp or .htm page that is popping up should not be a problem. On my sites, my pop-ups pop up an .asp page. My suggestion would be to put the FULL path in the area where you have to put the page address (javascript and button code) like this: http://www.wesellstlouis.com/test/residreq.htm I should have put that in my first post. Sorry. I tried your page and the pop-up pops up. When I do this type of work, my first goal is the get the page to pop up in the window correctly (with some test text in it). Once you know that works, then put the form in. Keep it HTM.

Are you aware of the procedure to send multiple emails using FP 2000 confirmation form?
