I found a shopping cart script I was going to try to use: Question - I want to set up an order form for the visitor to fill out for personalization of the product, use the shopping cart to add the totals, and send a confirmation notice back...but how does with the touch of one button "add to shopping cart" does the client get the order form via e-mail, send a confirmation, and the 3rd party payment processor (paypal) get the totals?

Normally the one button "Add to cart" AmazonŠ is only part of the process.
You will still have to fill in all the forms as you suggest. Each cart is different

I was looking for a little more info. So can one push of the button do 3 tasks: send order form to the merchant who fulfills it, send a confirmation to the customer, and send a notice for funds to PayPal?

That depends on the Cart/Script. If it's configured to - yes.
