I am using categories and page titles from each page or document's properties to generate a TOC. I renamed a file holding documents and now each of those documents appears twice in the TOC - once with the proper title and once with the old file path. How can I get rid of the one's listed with the old file path? The documents only exist once within the website.

Have you tried to recalculate hyperlinks?
