This is simple really, but I see it done the wrong way - all the time. Myself included.

1. Have a distinct <title> for each page. The title should explain in a few short words (your keywords) the intent of the page. Don't put your company name first - place your keywords first and if you feel you absolutely must have the company name in the title add it second.

I think what often happens is webmasters make a template for the site and recycle it over and over. When they remember, they will change a few tags, but most concentrate on the on page spam - quality content.

2. Have distinct <keywords> and <description> tags for each page.
If each page has different content then your meta tags should indicate this difference. Just like the <title> tag, reflect the content. Change the words around enough so that you can hit various typings of your keywords.

These changes are small, but they can make a real difference in building long term traffic. Most sites traffic come from the less popular phrases that the site owner optimized for - in fact, more often than not the site owner did not obtimize for the bulk of their traffic. This bulk is refered to as the Tail. Having proper <title> and <keyword> <description> tags can really help reel in traffic.

Just one thing - Make sure you mention within the <body> the topics you include in the <title><keywords><description>. If the title mentions 'long green widgets' you should also talk about them somewhere, but mentioning 'short green widgets' is also important.

For those that keep score, yes, I have changed my position on the meta tags. I no longer believe they are completely worthless. If you believe you can help rank for competitive terms using meta tags I still think you're stoned, but grabbing less popular terms that fly under the algo filters can be strengthened with proper usage of meta tags.
