You can use Front Page to create the database if it will remain fairly simple.  Create a new page, insert a form with the fields you want to include in the database, and then right click in the form and choose Form Properties.  On the pop up property sheet, choose Send to a Database... and then Options.  On the new window that opens, one of the options is "Create Database".  Of course, I've always used that on a machine that has Access, so I'm unsure if that will work if Access isn't on the machine.  If it does work, you can add new fields by creating another form, going in the same way, but choosing Update Database instead of Create Database.

The database will be named the same as the file name of the form you use to create it, so make sure to save it as the name you want the database to be.

Not really - although you can use asp to create a database file (long and involved really)
Its probably best to find someone who has Access?

There are scripts out there that will allow you to edit Access DB's via a web browser but:

1.  You have to have a file there to begin with.  That's easy enough since FP will build a basic file for you.

2.  Some of the scripts cost as much as Access itself.  :)

3.  Some are better than others.

Here might be a good place to start:
