Found this in a search. They explain quite a bit about the options you have in crafting your .htaccess file. It doesn't answer all questions but I thought it might be helpful to someone.

Yes, I found JavaScriptKit too and their info is very helpful. But I have avoided that site because I really hate those advertising links that get put into the content. I don't like feeling I'm being tricked into clicking on an ad, and I don't like having to be that vigilant to keep track of which is an ad link and which is a "real" link. (grrrr...)
