What do you mean what am I using?

To design? Notepad or HTMLKit.

Standards compliant XHTML and CSS with media="handheld" stylesheet.

Have fun. You need a fluid site as they're all different widths from 300px wide down to approx 120px.

Remove images too costly to download.

DW8 supports "XHTML Mobile 1.0" format, I didn't see any support from FP2003.

When saving your page, do you use index.html or index.xhtml?

XHTML uses the same .html file extension as other versions of HTML.

I doubt if mobiles are worth farting around with. 3G phones which were heralded as the next big web thing have been a dismal failure (in the UK at least) virtually nobody is using them to access the internet, they are mostly being used to receive and make phone calls. The companies that invested such a huge amount of money in setting up 3G web services (mainly paying the government inflated prices for the licenses) are all losing a fortune on it. Consumers are not prepared to pay through the nose to use these very small screens. I believe it is probably worth setting up a style sheet to produce a very simple version of your web site i.e. just one page with no pictures and only contact details and leave it at that.

With a handheld stylesheet you can have your whole site "mobilified" rather than bother with a separate single page site for mobiles - just, as you say, simplified - no images for instance. Not all mobile devices understand handheld stylesheets - but more and more do.

I use .htm mainly because I'm too lazy to type all the extra l s. :)

Most hosts accept that. If not and you're on Unix hosting you can use .htaccess to tell the server to parse any extension you want as any other extension.

I wouldn't make a separate page but I would use the small screen sheet to limit the site to one page with contact info only. IMHO (and the limited take up of web services on hand-helds backs me up) there's no point in worrying too much because virtually no one is going to be using a hand held device to browse your site.

The only bug I've found is in Opera and that was when I was using the small screen renderer to check out a .mobi web site. :)

To be honest, given the IE track record, I'd rather put my faith in FF. Sure it has some bugs, no software that complex will ever be completely clean but as has been said, when bugs are found they get fixed fast whereas the IE bugs can take months. Don't get me wrong, I think IE7 is a huge improvement over what's gone before but let's face it, it's a patched IE6. You've got to wait for IE8 for new architecture and lets not forget that MS are now working with the Mozilla guys.

Far as I've read even IE8 won't understand XHTML so it ain't going to be that different!

Anyone who says FF doesn't have bugs is fooling themselves - the difference is that they get fixed, fast.
