Depends what sort of forum you're running. phpBBhacks.com has loads of stuff for phpBB forums (though I've used some stuff from there on other forum systems), there's various button sets and stuff linked to at SMF (mainly links on the forum.
One of the best icon sets I've come across is the FamFamFam Silk icon set, but there are loads around. I've found various icon sets and buttons and stuff simply by googling. I tend to use combinations from various icon sets and if I can't find what I'm looking for them I set to and make my own.
