Did you publish the FP website to your website?

The form is up and appears normal but I don't see a submit value statement in your code. Example:

<input type="submit" value="Send to xxxxxxxxxxx"

Make sure you have completed Form Properties with your email address for the Submiit field (right click) then answer No in the drop down box after entering OK.
