I check the code on your form page and you have a reference to the results file as follows: U-File="C:\Documents and Settings\Owner\My Documents\Toffee Mom\_private\formrslt.htm" As you can see, the file path is showing that as on your PC and not on the server.

Hi William, Thanks for the response. I tried to update the code to: U-File="_private\formrslt.htm" And just that change made the entire form disappear when I uploaded it to the site. It was still showing in FP but not when I would upload. Strange no? I also went into Form Properties and specified the file there but I still received blank emails. And I checked the results file and it was empty too. Any other thoughts? TIA

Did you use Publish web or you use FTP?

Yes to all. I have FPSE on my server. I've also uninstalled and reinstalled them. I'm Publishing from FP I think I'm working in a FP web. By that do you mean a "site" with all of the folders in Front Page?

You are probably working in a disk-based web. OK, do this. In FP, go to Open>New web or New>web then choose an EMPTY web. In the FILE menu, go to IMPORT and browse to your toffeemom folder in My Document. Import the whole folder. Then publish from this web.
