meinthemail -> Upload File Box requires Username & Password??? (3/7/2008 15:26:09)

Hi, I've been intouch with my host and they say my problem is with programming so here I am! My forms work ok on my website that is until i add a "file upload box" then when my cumstomer clicks submit a pop up window appears saying "the server at myname.come requires a username and password..."
Incidentally I try entering my login deatails and get declined???
How can I make it so customers can just upload their photos?
