Should a header image have an alt tag? If so should it be empty <img alt=""/> to avoid having the same content at the top of each page? Thanks!

yes it should and it should represent the image in text - so if it's a logo/header for 'Custom Embroidery and Screen Printing' then the alt should read 'Custom Embroidery and Screen Printing'. and it should be wherever the image appears.

The alt tag for the image doesn't have great impact for SEO but it is better to put it so your visitors can get more information about the image.
